Current Positions Available

Medical Social Worker, F/T

Medical Social Worker’s are responsible for providing social work services to Hospice patients and their families in a variety of “home” settings.  The MSW will also assist with community resources, counsel patients/families on end-of-life issues, and work with the interdisciplinary team.  The successful candidate will have a Master’s degree in Social Work and/or LCSW, as well as one year of social work experience. 

RN Case Manager, F/T  

RN Case Manager’s are responsible for compassionate end-of-life care which conforms to the Hospice patients/families wishes.  Visits are in patients’ homes, assisted living facilities, and skilled nursing facilities.  One year RN experience req.  Hospice/Home health experience preferred. 

Home Health Aide

This full-time, licensed position performs visits and delivers personal, practical care for Hospice patients in their homes. The Hospice Home Health Aide also provides assistance with activities of daily living and some homemaking duties.  CNA and HHA certificates are required. 

Quality Assurance Nurse, P/T
This part-time position assists the Quality Assurance Nurse Manager in implementing Quality Assurance/Performance Improvement activities in identified patient care and support departments. This position will be responsible for assisting the Quality Assurance Nurse Manager in education of staff about Quality Assurance/Performance Improvement related issues.  Home Health or Hospice experience preferred.
