Medical Social Worker, F/T

Medical Social Worker’s are responsible for providing social work services to Hospice patients and their families in a variety of “home” settings.  The MSW will also assist with community resources, counsel patients/families on end-of-life issues, and work with the interdisciplinary team.  The successful candidate will have a Master’s degree in Social Work and/or LCSW, as well as one year of social work experience.  Please fax resume to: (805)782-8614.  Application may be downloaded from our web site.

RN Case Manager, F/T  

RN Case Manager’s are responsible for compassionate end-of-life care which conforms to the Hospice patients/families wishes.  Visits are in patients’ homes, assisted living facilities, and skilled nursing facilities.  One year RN experience req.  Hospice/Home health experience preferred.  Please fax resume to: (805)782-8614.  Application may be downloaded from our web site.
